COSMETIC SURGERY

A surgical procedure undertaken to reshape and change the appearance of a body part in accordance with personal aesthetic outcomes.

Abdominoplasty +

There are many reasons why people are not happy with their tummy area which may be as the result of pregnancy, weight loss or gain, ageing, or their own natural shape. These changes can be related to problems of the skin (looseness, stretch marks, irritation dermatitis or intertrigo), problems of fat and fat distribution (uneven fat distribution, loose apron, contour changes, buttock volume loss), and changes to the muscle (loss of muscle tone, muscle separation called rectus divarication). There are several techniques to address individual concerns and conditions, which Mr Yang considers with your optimal outcome in mind, and when changes can no longer be made through diet and exercise.

Most of the changes in the tummy’s appearance are commonly most pronounced centrally in the abdomen, and an abdominoplasty is a common procedure that addresses these changes. Abdominoplasty is designed to remove excess fat and skin, while surgically tightening sagging muscles to improve the abdominal wall’s function and appearance.

Addressing changes following severe weight loss

Many of these changes can also occur following significant weight gain and subsequent weight loss and are frequently greater when pregnancy is associated with weight gain and weight loss. Under these conditions changes occur in multiple areas of the body and it is common to address changes in more than one area at the same time. These are often referred to as massive weight loss (MWL) surgeries. In some media the term ‘mummy makeover’ is used when breasts and body are addressed together. These problems can occur in both men and women with significant weight loss. MWL patients often have more substantial concerns although the abdomen is usually still the most affected area. The difference is that the looseness goes all the way round the body rather than simply in the abdomen. Consequently many of these patients will consider a circumferential waist tuck, sometimes incorporating a lower body lift that addresses the abdomen, flanks, outer thigh, buttocks and lower back rather than just the abdomen. A number of different procedures may be considered as part of massive weight loss surgeries. How they may be combined safely into the same surgical session or staged as separate surgeries will need to be individualised following careful assessment and planning.

Arm lift (Brachioplasty) +

An upper arm lift reduces and reshapes the tissues hanging from the under-surface of your upper arms when the arm is held out from the body. This is an area that can be unsightly and unfortunately attracts a number of unflattering descriptions such as ”bat wings”. The problem is generally worse in people who have lost significant amounts of weight, but can also develop with ageing. Liposuction is used in combination with skin reduction to achieve the best outcomes.

Am I a Candidate?

You may have a combination of physical and aesthetic concerns from the excess tissue on your arms:

  • Chafing (intertrigo) - this create challenges for skin care and disrupt daily activities and exercises due to rubbing and discomfort.
  • The shape of the arms is dominated by underarm skin laxity.

Suitable candidates are people who:

  • Are in good overall health and have a stable weight.
  • Have no serious medical issues.
  • Have realistic goals and expectations
  • Understand the risks of surgery, the likelihood of complications, and what is required to manage them.
  • Are knowledgeable about what it is required to achieve the best outcome from surgery both in preparation for surgery and recovery from surgery.

Blepharoplasty (eyelid lift) +

With ageing or hereditary factors, saggy eyelids commonly occur and are the source of dislike for many people (hooding in upper eyelids; eye bags in lower eyelids). Blepharoplasty is one of the most regularly performed surgical procedures, where excess skin and sometimes fat, is removed from either the upper or lower eyelids, or both.

In some instances, the upper eyelid itself is drooping (eyelid ptosis) and is masqueraded by excess upper eyelid skin. During an upper blepharoplasty procedure the eyelid position can also be corrected by tightening the eyelid opening mechanism (ptosis surgery).

For lower eyelids, eye bags may be exacerbated by loosening of the eyelid through the natural process of ageing, allowing them to turn outwards (ectropion) and causing tears to pool and not drain properly. During a lower blepharoplasty procedure, the lower eyelid can also be tightened (ectropion surgery). Sometimes a mid-face lift may also be combined to further bolster the support to a loose lower eyelid.

The blepharoplasty procedures can significantly improve and refresh the overall shape and look of the eyes. While these are usually cosmetic procedures, sometimes excess skin, significant upper eyelid ptosis, or severe lower eyelid ectropion can interfere with vision, in which case some medical insurers may contribute to costs. The surgery restores a more youthful appearance, the downtime is minimal and the procedure can be done under both general and local anaesthesia.

The benefits from blepharoplasty may include:

  • Restoring youthful definition to your eyelid
  • Removing excess skin and fat
  • Reducing lines and wrinkles under and around the eyes
  • Correcting a droopy upper eyelid position, eyelid ptosis

You may be a suitable candidate for upper eyelid surgeries if:

  • You are concerned about drooping eyelids, excess eyelid skin, eyelid bulges or hallowing, or eyelids obstructing your vision
  • You have good health and realistic goals and expectations with regards to the surgery
  • You do not have problems with chronic dry eyes or persistent watering of the eyes
  • You are not suffering from recurrent active bouts of upper eyelid swelling (oedema)
  • You are not currently having treatment for an eye disease through an eye specialist

You may be a suitable candidate for lower eyelid surgeries if:

  • You are concerned about lower eye bags, or out-turned lower eyelids with pooling of tears

Your surgeon may advise combining an upper eyelid blepharoplasty with the following:

  • Brow/forehead lift - brow positions can drop with forehead ageing to compound the appearance of excess folds in upper eyelid skin, giving a false impression of a constantly tired and/or angry appearance
  • Face lift

Your surgeon may advise combining a lower eyelid blepharoplasty with the following:

  • Lower eyelid ectropion surgery
  • Mid-face lift

Double eyelid surgery (Asian Blepharoplasty)

Many Asians are born with eyelids that are naturally without a crease (“single eyelid”). Asian blepharoplasty is a cosmetic procedure in which the eyelid is reshaped and an upper eyelid created with a crease (i.e. "double eyelid").

Epicanthoplasty (Asian eye widening)

The medial epicanthal fold is a unique feature to the Asian eye, stretching over the inner (medial) corner of eye where the upper and lower eyelids meet. Medial epicanthus greatly affects the appearance of the eye. Epicanthoplasty is a procedure that realigns skin folds at the inner corner of the eye to make the eyes look larger and clearer. The procedure is often performed with the Asian double eyelid surgery to maximise the enhancement to eyelid appearance. It requires a series of small incisions at the inner corner of the eyelids. Mr Yang takes great care to minimize scarring through a conservative approach with fine instruments. Mr Yang received fellowship training in Korea for double eyelid surgery. Brow lift +

A brow lift gives a person a more refreshed, and less ‘angry’ look by decreasing the drooping and wrinkling around the forehead and eyes. Drooping of the brows occurs with age, but frequently a low brow can appear to sit in a normal location because the muscle of the forehead constantly tries to elevate the brow to assist with vision, creating the horizontal wrinkle lines across the forehead.

The benefit from brow lift may include:

  • Elevate the brow higher towards a more normal position
  • Improve forehead wrinkles
  • Lessen frown lines around eyes and near nose

You may be a suitable candidate for brow lift surgery if you:

  • Have a heavy feeling in the forehead and a low sitting brow
  • Have forehead wrinkles
  • Are realistic about what you want and can be achieved

Breast augmentation +

Am I a Candidate?

A breast enlargement may be suitable if you want to increase the volume and improve contour or balance of your breasts.

Suitable candidates are women who:

  • Are over 18 years old. (Breasts may continue to grow up until the age of 18.)
  • In good overall health and at or near their goal weight.
  • Have no serious medical issues.
  • Is knowledgeable about how the procedure is administered.
  • Understands the limitations of artificial breast implants.
  • Appreciates that both breasts, nipples and areola are never exactly the same size or shape.
  • Have breasts without unexpected lumps, bumps, or firmness.

Breast lift (Mastopexy) +

For women who are happy with their breast size, but desire a firmer, lifted appearance. Occasionally a small silicone implant is used to enhance shape.

Am I a Candidate?

A breast lift may be suitable if you are satisfied with the proportions of your breasts and cup size but are unsatisfied with where your breasts sit.

Suitable candidates are women who:

  • Have breasts that are sagging, but satisfactory in size.
  • Are over 18 years old. (Breasts may continue to grow up until the age of 18.)
  • In good overall health, and at or near their goal weight.
  • Has no serious medical issues.
  • Has areolas and nipples that point downward.
  • Is knowledgeable about how the procedure is administered.
  • Have breasts without unexpected lumps, bumps, or firmness.

Brow lift +

Facial contouring +

Face lift +

Depending on the individual and what they wish to address through this cosmetic surgery, upper, mid or lower facelifts are performed. During your consultation, Mr Yang will assess your face and discuss with you just what is needed to provide you with the results you seek. Many people combine the facial procedure with an eye or brow lift and micro fat grafting to restore volume and shape. Facelifts today are about obtaining a fresher more youthful tone, not the taut, shiny and unrealistic result of days gone by. Scars are hidden behind ears and in natural creases and fade over time. The procedure is done under general anaesthetic at either Tristram Private Clinic, or Braemar Hospital.

Fat-grafting (Lipomodelling) +

This procedure is used primarily to transfer volume (fat) from one area of your body to another. Typically the fat is taken from the tummy and thighs and transferred to breasts, chins, cheeks and forehead. Small top ups may be required to achieve the desired outcome. Fat grafting requires detailed planning with specific goals in mind, and Mr Yang will make a careful assessment to determine how this procedure might work for you.

Labiaplasty +

Performed for both reconstructive and/or cosmetic reasons, this procedure is concerned with reducing the size of elongated labia in women and can be performed under either local or general anaesthetic. The condition may occur due to damage during childbirth and reduces discomfort caused by this.

Am I a Suitable Candidate?

Women experiencing different degrees of labial hypertrophy or elongation with irritation, are often suitable candidates for this surgery. Most patients want labiaplasty surgery to decrease the size of their labia minora. Suitable candidates are women who:

  • Have concerns about the appearance of their labia.
  • Have a large labia and loose minora.
  • Are emotionally and physically healthy.
  • Have realistic expectations.
  • Are wanting a Labiaplasty for their own reasons, not someone else’s.

Liposuction +

Often referred to as ‘lipo’ or ‘liposcupture’, this procedure is commonly used in cosmetic plastic surgery. Fat is removed from areas such as neck, ankles, back, abdomen, thighs, hips and buttocks, when diet and exercise have reached their efficacy, or to achieve contouring or sculpting in a specific area. Because a cannula (hollow thin tube) is used to suction the fat, the work can be very precise. This is not a weight loss treatment, and it does not treat stretch marks or cellulite. Liposuction is performed under general anaesthetic and compression garments may be advised for optimal results.

Neck lift +

By surgically tightening loose skin, lines and wrinkles will resume a smoother, more youthful appearance. Liposuction may be used to complement the procedure, removing any excess fat under the chin and jowls, contouring and defining the jaw line. It is often most effective when combined with lower face lift.

Am I a candidate?

There is no age limit for when facelift and neck surgery should be administered; although the most suitable candidates are between 40 and 65.You may be a suitable candidate if:

  • You have noticeable signs of facial ageing (skin laxity/sagging skin)
  • You are near or at your ideal weight (excessive weight gain or loss will affect the results of surgery)
  • You are not a smoker, or have stopped smoking for at least 4 weeks

Thigh lift +

Rhinoplasty +

Rhinoplasty - often referred to as a “nose job” - may be performed for either cosmetic or functional purposes. Mr Yang is passionate about this procedure, which is regarded as one of the most challenging branches of facial aesthetic surgery. He has a keen eye for detail and is fastidious about symmetry and realistic outcomes. Rhinoplasty deals with facial balance and breathing functionality. Nose surgery remodels the shape of the nose and achieves a more desirable look by eliminating bumps and asymmetry in both front and side views. The end result is a more symmetrical and balanced nose that complements other facial features. Rhinoplasty may be performed following trauma, or to refine and/or improve nose deformations ranging from minor to severe (such as seen in congenital cleft lip and palate patients). Mr Yang received fellowship training under the auspice of international rhinoplasty master Professor Nasim Cerkes and attends international rhinoplasty courses and symposiums to keep up to date with the latest rhinoplasty concepts and techniques shared by international masters in the field.

Am I a candidate

The nose normally matures by the age of 17-18 years. A rhinoplasty is not usually performed before this age, as further nose growth causes unwanted shape changes.

You may be a suitable candidate if:

  • You have noticeable nose hump, bulges, shapes or asymmetry that you would like improved
  • You have a birth deformity affecting the nose
  • You had damage to the nose or its internal structure due to injury, which may have also restricted the breathing passages
  • You are not suffering from chronic nasal diseases
  • You are over 18 years old
  • You are in good overall health
  • You are wanting it for your own reasons, not someone else’s or for secondary gains

Weight loss surgery +

Body contouring surgery is not a single type of surgery, but an approach which utilises different surgeries and techniques to contour various parts of the body by removing or tightening excess skin, often after significant weight loss. The most common areas to benefit from these surgeries are the waist, abdomen, thighs, buttocks, upper torso and upper arms. With an increasing number of people affected by obesity, so too are more people motivated to undertake the difficult task of losing massive amounts of body weight to address their related health issues. Many have managed with successful dieting, while others choose bariatric surgery. Unfortunately loose skin on various parts of your body often linger after massive weight loss, causing rubbing and irritations with movement and exercises. Breast Reduction +

Many people view breast reduction as a cosmetic surgery, but this could not be further from the truth. Lifestyle and everyday activities can be severely limited in people suffering from large and heavy breasts, ranging from neck/back strains to skin care issues, difficulties with clothing as well as social stigma. Mr Yang believes breast reductions to be a functional surgery, and many of his patients describe it as life changing. A large breast that is also sagging may benefit from a combined breast lift. Mr Yang was trained in different breast reduction techniques and aspires to aesthetically pleasing breast shapes after surgery.

Am I a Candidate?

A breast reduction may be suitable if you have large and heavy breasts causing one or more of the following:

  • Pain, especially around menstruation.
  • Causing strains on your neck and back
  • Restricting physical activities

  • Suitable candidates are women who:
  • Have breasts that are large and/or heavy.

  • Breast feeding is not an absolute future consideration.

Carpal Tunnel +

Carpal tunnel is a ‘tunnel’ in the wrist made up of bones and ligaments. It supports tendons (which move the fingers), and the median nerve (provides feeling and movement) from the forearm to the hand. Numbness and tingling of the hands/fingers can occur if there is any swelling to the already tight tunnel – due to pregnancy, injury, inflamed tendons and so forth. If non-surgical methods are not successful, a carpal tunnel release is performed under local or general anaesthetic, which effectively divides the carpal ligament and release pressure.

Facial Cancer Surgery +

Gynaecomastia +

Gynaecomastia is swelling of the glandular breast tissue affecting 30-40% of boys or men. It is caused by an imbalance of hormones which can sometimes be a temporary phase through parts of normal growth in the newborn, boys during puberty, and ageing in men (physiologic). Teenagers may sometimes grow out of physiologic gynaecomastia over a couple of years, or by age 20. However, other causes also exist including liver disease and medications. Persistent male breast enlargement, especially if occurring with pain and discharges, should be reviewed by a family doctor and/or a hormone physician (endocrinologist). It is important to undergo the appropriate investigations and receive corrective treatment (including medications) if warranted. Men and boys with gynaecomastia can have discomfort in their breasts and may feel embarrassed or even distressed. Those with gynaecomastia who have normal or corrected hormone levels may consider gynaecomastia reduction surgery if recommended by your family doctor or hormone physician. Am I a Candidate?

A male breast reduction may be suitable if you are concerned about enlarged breasts in one or both sides, persisting more than two years, despite normal or corrected hormone levels and health checks by your family doctor or endocrinologist.

Suitable candidates are males who:

  • Are in good overall health.
  • Are knowledgeable about how the procedure is administered.
  • Have realistic expectations about what the surgery can achieve.
